High-resolution quantitative spiral perfusion for microvascular coronary dysfunction detection

چکیده
Background Quantitative first-pass CMR can assess the severity of microvascular coronary dysfunction(MCD) in subjects with signs and symptoms of ischemia but without obstructive coronary artery disease(CAD). In patients with MCD, abnormalities of myocardial perfusion reserve (MPR) occur globally but may also vary in their transmural extent. Thus, high spatial resolution quantitative CMR perfusion imaging is needed to differentiate between the subendocardial and subepicardial layers. Given the efficiency and SNR of spiral trajectories, we have developed the accelerated high-resolution absolute

Reliable detection of subendocardial ischemia by high-resolution end-systolic first-pass perfusion imaging in the absence of obstructive coronary disease
Background Among the patients without known disease undergoing invasive angiography, nearly 40% do not have coronary artery disease (CAD) [1]. A possible underlying etiology for such patients is ischemic heart disease associated with coronary microvascular dysfunction (CMD). Despite intense interest and recent advancements, reliable diagnosis of CMD on the basis of first-pass perfusion (FPP) ca...
متن کاملAdenosine stress CMR with spiral pulse sequences accurately detect CAD
Background Adenosine stress perfusion imaging with CMR has been limited by motion-induced dark-rim artifacts, which may be mistaken for true perfusion abnormalities. We have previously demonstrated that spiral pulse sequences can produce high quality first-pass perfusion images.[1] We have further improved this spiral technique using high-resolution variable density spiral trajectories and a no...
متن کاملComparison of fully quantitative and semi-quantitative measure of women's myocardial perfusion reserve for detection of microvascular coronary dysfunction
Background Myocardial perfusion reserve (MPR) acquired using firstpass cardiac MRI is a measure of the severity of microvascular coronary dysfunction (MCD) in subjects with signs and symptoms of ischemia but no obstructive coronary artery disease (CAD) [Shufelt 2011]. Semiquantitative measures (upslopes) have been used to analyze MPR data in the previous work [Shufelt 2011]. In this work, we pr...
متن کاملImaging Microvascular Dysfunction and Mechanisms for Female-Male Differences in CAD.
Microvascular dysfunction or disease is most commonly associated with diffuse epicardial coronary atherosclerosis and endothelial dysfunction, whereas it is less common as a distinct, separate, isolated pathophysiology. The different manifestations of coronary artery disease in women relate in part to their smaller coronary arteries, higher coronary blood flow, and higher endothelial shear stre...
متن کاملRelation between stress-induced myocardial perfusion defects on cardiovascular magnetic resonance and coronary microvascular dysfunction in patients with cardiac syndrome X.
OBJECTIVES The purpose of this study was to investigate whether a direct relation can be demonstrated between myocardial perfusion defects detected during dobutamine stress test (DST) by cardiovascular magnetic resonance (CMR) and impairment of coronary microvascular dilatory function in patients with cardiac syndrome X (CSX). BACKGROUND Despite the fact that coronary microvascular dysfunctio...
